What is free testosterone?
Free testosterone is the small fraction of testosterone circulating unbound in the blood that is available to bind to receptors and produce biological effects, as opposed to testosterone bound to carrier proteins.
Free testosterone refers to testosterone molecules that are not attached to carrier proteins like sex hormone-binding globulin (SHBG) or albumin. While total testosterone measures all testosterone in the bloodstream, free testosterone represents only the unbound portion that can actually enter cells and trigger the hormonal effects your body depends on.
Most testosterone in the blood is bound and inactive. The amount of free testosterone typically accounts for 1-3% of total testosterone, yet this small fraction drives the physiological responses that matter: muscle growth, bone density, libido, energy levels, and mood regulation. Two patients with similar total testosterone levels may experience very different symptoms if their free testosterone differs significantly.
TRT clinics often order free testosterone tests alongside total testosterone to get a complete picture of a patient's hormonal status. Total testosterone can be misleading on its own, particularly in men with high SHBG levels from age, weight gain, or certain medications. Measuring free testosterone helps providers determine whether a patient's symptoms align with actual bioavailable hormone levels and whether treatment adjustments are needed.
Free testosterone can be measured directly through equilibrium dialysis (the gold standard) or calculated using total testosterone and SHBG values. Most Florida TRT clinics order both total and free testosterone tests during initial assessment to establish a baseline and inform dosing decisions.
